To register for our online services you will need to download and complete the following form(s) and email esccg.smallfield@nhs.net with copies of your ID, or bring them to the surgery:

• Application for Online Access (over 16)
• Application for Proxy Online Access (over 16)
• Application for Proxy Online Access (young person 13-16)
• Application for Proxy Access (child under 13)

Once you are registered you will be able to use the service to:

• Order your repeat prescriptions
• Make an appointment
• Cancel an appointment
• Change your contact details
• Review your medications and known allergies
